Community Outreach

 

Community Outreach provides Attendant Services to individuals with a physical disability 16 years of age and over, who are living in communities throughout the Kenora and Rainy River Districts.

Services are available on a scheduled basis, 7 days a week (including holidays) within a 24 hour period.

HOURS OF OPERATION

To assist consumers with their daily living requirements, the program operates 7 days a week (including holidays), 24 hours a day.  Consumers can schedule assistance for 15 minutes per day up to a maximum of 6 hours per day and can receive from one visit per day or week, up to 5 or 6 visits per day, 7 days per week.

CONSUMER OPTIONS

Two choices are available to consumers regarding the management of staff:

1. In conjunction with the Coordinator, the consumer can hire and manage their own attendant staff person or,

2. The Coordinator will provide and manage the attendant staff person.
